Population Health Alliance Partners with Accountable For Health.

WASHINGTON, Dec. 6, 2023 "Value-based care is an important facet of Population Health Improvement as the programs align priorities and incentives to achieve the best outcomes for consumers," said Rose Maljanian Chairman of the Board and Chair Emeritus, Population Health Alliance (PHA) and Chair & CEO HealthCAWS. As such, the Population Health Alliance has partnered with Accountable for Health (A4H) to advance Value-based care policy in areas important to its members.

Population Health Alliance members represent health systems, ACOs, health plans, employer solutions across commercial, Medicare and Medicaid lives who have dedicated patients and members for whom they are accountable. The PHA Population Health Management Framework starts with leveraging data and technology to identify individuals in need, followed by triage to the best intervention at that time in the individual's health journey, and then measuring outcomes iteratively to achieve the best possible outcome for the individual, vulnerable population subsets, and the population as a whole.
